About the service:

The Autistic Spectrum Disorder (ASD) Service comprises of two teams.

  • Pre-School ASD team
  • School Age ASD team

The pre-school team see children up until their 5th birthday and the School Age team deal with children and young people from five years until their 18th birthday

We provide:

  • Comprehensive assessment to explore whether a child or young person meets the criteria for a diagnosis of ASD. This assessment will take place over a number of appointments and will include collecting information from parents and school, as well as a play-based assessment with the child or young person who is being assessed.
  • With parents consent, information will be sought from other professionals who have worked with their child and a school observation may be carried out. Parents will be provided with a written report following the assessment.
  • A post-diagnostic psychoeducation workshop for parents following their child receiving a diagnosis of ASD.

Referral criteria

We accept referrals of children and young people up to the age of 18 where there are concerns regarding social interaction, communication, and restricted and repetitive behaviours.

We also accept referrals for the post-diagnostic workshop for parents of children and young people who receive a diagnosis from another service.

Referral routes

  • Professionals working in Child & Adolescent Mental Health Services (CAMHS) providing that an initial assessment has been carried out and further specialist assessment or intervention by the School Age ASD team is recommended.

  • Speech and Language Therapists
  • Parents of children and young people who have already received a written diagnosis of Autistic Spectrum Disorder are able to contact the service direct if they wish to receive some further intervention

How should a referral be made?

Referrals from professionals should be made in writing containing sufficient information to allow the team to understand why the referrer believes an ASD assessment is appropriate. All referrals should contain informed consent from the child and/or those holding parental responsibility.

We work with children and young people who are experiencing significant difficulties with their emotional wellbeing/ mental health, or where there are concerns regarding their development, which could be consistent with a neuro-developmental disorder such as Autistic Spectrum Disorder (ASD) or Attention Deficit and Hyperactivity Disorder (ADHD), assessment for children over 6 years of age.
 

What we do

We offer an assessment to understand a child/ young person’s difficulties and where appropriate deliver specialist interventions guided by evidence-based practice and guidelines.  We offer a range of therapeutic options including:
  • Psycholgical assessment.
  • Neuro-developmental assessment.
  • Cognitive Behavioural Therapy (CBT) for difficulties including anxiety and depression.
  • Dialectical Behavioural Therapy (DBT) for young people having difficulties regulating their emotions.
  • Behavioural work for young people experiencing phobias, anxiety and low mood.
  • Systemic Family Work.
  • Eye Movement Desensitisation Reprogramming (EMDR).
  • The New Forest Programme for parents of young children diagnosed with Attention Deficit and Hyperactivity Disorder (ADHD).
  • Triple P Parenting Programme.
 Please note: We do not offer counselling services, as our partnership agencies have the expertise to deliver a counselling model and are therefore best placed to see children requiring this intervention.
 

Who we work with

We can assess children who have a mild learning disability.  Children with a diagnosis of moderate to severe learning disability will be referred to the CAMHS LD Team for assessment.

We will work collaboratively with other agencies involved in children/ young people’s care including Education, Health and Social Care.

We are a child and adolescent community mental health team consisting of staff from a range of backgrounds including Mental Health Nurses, Cognitive Behavioural Therapists, Psychiatrists, Psychologists, Mental Health Practitioners, Play and Parenting Specialists, Occupational Therapist and Administrators. All staff are employed by the NHS.

Child and Adolescent Eating Disorders Service

About the service

We are a service that works with children and young people aged 8 to 18 who have a diagnosis of an eating disorder such as anorexia nervosa or bulimia nervosa and atypical eating disorders. 

Our team is made up of a consultant psychiatrist, consultant paediatrician, dietician, therapists and mental health practitioners, health care support workers and our admin who keeps everything together.

What we do

Our aim is to support young people and their families to recover from their eating disorder by learning what is keeping the problem going, provide a space to talk about their fears and worries and find ways to manage which are not harmful to them.

We use a number of different therapies to support our young people and their families, to get better including Family Therapy, CBT-e and where appropriate our psychiatrist will look at medication that can help with mood and anxiety.

 We offer individual work, dietetic input, family therapy and we also have a parent/ carer group that occurs once a month.  Our health care support workers provide meal support to those who need a bit more help with improving their eating pattern and this can be at home or in school as necessary.

About the service

The Darwin Centre is a regional 15-bedded inpatient unit, providing specialist mental health services for young people and their families. The centre is open 24/7. The catchment area covers Staffordshire, Shropshire and the West Midlands.

The primary function of The Darwin Centre is to provide assessment and treatment facilities for youngsters with mental health disorders (including deliberate self harm, acute psychosis and eating disorders). Youngsters are admitted for completion of a comprehensive multi-disciplinary assessment which involves developing an understanding of their mental health and psychological wellbeing, in the context of their developmental, social, family and educational history. This includes work with young people and their families at all stages of their treatment.

The service provided is for young people within the age range of 12-18 years. Exclusions due to physical, mental or behavioural illness or disability cannot be specified and are assessed on an individual basis. However, young people with moderate to severe generalised learning disabilities, those under a secure care order and those whose primary difficulty is substance or alcohol misuse, will not normally be treated at the Centre.

Referral criteria

Referrals are taken from CAMHS Tier 3 community teams, Early Intervention teams and dependent on age, adult mental health teams.

About the service

This is a specialist service for children and young people (aged 5-18 years), with a diagnosed severe learning disability and additional complex health needs, which require Registered Nursing Care whilst the child is away from home. These needs may either include severe and significant challenging behaviour, which could not be managed in other respite settings, or medical/physical conditions requiring regular and frequent nursing interventions.

Children and young people accessing the service will reside in North Staffordshire i.e. Stoke-on-Trent, Newcastle-under-Lyme or Staffordshire Moorlands.

About the service

The Early Intervention Team is a dedicated service for those aged 14-64 years. It supports service users and families affected by a first episode of psychosis or at risk mental states. The team are implementing the access and waiting times standard by delivering NICE concordant interventions in a timely way.

The service vision is:

  • To improve the life chances of those affected by a first episode of psychosis.
  • To provide a person centred recovery approach that responds to the needs and aspirations of those we work and engage with.
  • To reduce the time it takes to provide effective treatment and support when a person first starts to become unwell.
  • To respond with compassion and care, instilling hope and optimism for the future.
  • To provide a service that delivers a socially inclusive approach strengthening users social capital.
  • To work effectively and creatively with users and partners to deliver recovery outcomes.

ELEVATE meets twice a month and work together on a range of projects to contribute to the success of our ever-developing services.

Not only this but ELEVATE is a space for young people to socialise and meet people from a diverse range of backgrounds, all with the same passion of developing services.

About the service

The new Mental Health Support Teams (MHST) will be working within educational provisions to deliver evidence-based early interventions for children and young people experiencing mild to moderate mental health problems.

What we do

The Education Mental Health Practitioners (EMHP), who are part of the MHST, are new roles providing early intervention to better address emerging mental health needs. Their role is intended to implement an effective whole school approach to mental health to improve outcomes, whilst also being able to provide brief, evidence-based interventions at an early stage of need and reduce the need for future specialist CAMHS interventions.

The rest of the team are made up of staff from other professional backgrounds such as social work, mental health nursing and child nursing.

The MHST can help signpost to services that may be better placed to meet the wide and varying needs of children and young people and their families without the need to access CAMHS.

About the service

The Paediatric Psychology Team provides a specialist psychological service to children, young people aged 0-18 and their parents and families where there are psychological issues related to acute or chronic physical illness or traumatic injury.

Team members undertake psychological assessment and intervention. They also work jointly with other professionals and agencies and provide support, consultation and training about psychological issues.

The service covers North Staffordshire. Referrals are also accepted for out of area children and young people whilst they are inpatients / receiving medical care at University Hospitals of North Midlands NHS Trust.

About the service

The Looked After Children’s CAMHS team provides a comprehensive therapeutic and consultation model of service delivery to the looked after children of Stoke on Trent and their professional and care networks. The LAC CAMHS team has close working and collaborative relationships with the Local Authorities ‘Children in Care’ and ‘Safeguarding’ Social Work teams from where it receives all of its referrals.

The team is comprised of Psychologists and Therapists who work closely with all community and specific CAMHS teams and have access and support from other colleagues such as Child and Adolescent Psychiatrists, Social Workers and Mental Health Nurses.

What we do:

We are a specialist CAMHS team that works closely together to make sure that children and young people who are in care have lives that are as psychologically healthy as possible. We work especially with children and young people who are experiencing difficulties with attachment and developmental trauma, either directly within a therapeutic relationship or often with their wider care and professional networks.

We offer a range of direct or systemic therapeutic interventions and also consultation, training and group work, some of which are listed below;

  • Psychological Assessments.
  • Specialist Consultations.
  • Marschak Interaction Method Assessments (MIM).
  • Theraplay
  • Play Therapy.
  • Dyadic Developmental Psychotherapy.

Who we work with:

We work with children and young people up to the age of 18 and their carers and professional networks where the referred person is subject to a Full Care Order, Interim Care Order or Section 20 of the Children’s Act. The referred person’s primary presentation should relate to concerns about attachment difficulties and developmental trauma or where there is a risk of current placement breakdown.

How you can refer to us:

All referrals to the service are made by the child or young person’s Social Worker who will be exercising parental responsibility on behalf of the Local Authority when a child is placed in care. We require a signed consent form from the child’s Social Worker or young person directly if appropriate and a completed SDQ form scoring 17 or over. Referrals are made through the Crisis Care Centre:

About the service

We provide:

  • A single point of contact for all Mental Health Crisis for individuals of all ages
  • A prompt and expert triage/assessment of needs
  • A multi-disciplinary team approach
  • Advice and support to service users, families and carers
  • A culturally competent service, including ready access to interpreter services for minority languages and British Sign language
  • A detailed understanding of the local population, its mental health needs and priorities, and a service that is sensitive to this and diversion and inclusion needs
  • Support and advice to primary care through collaborative working
  • Report the findings of assessments to referrers and other significant individuals in order to facilitate best care

Referral criteria:

Anybody who is experiencing a mental health crisis, recognising that a crisis is unique to all individuals.

This list is not exhaustive but gives examples; the All age Access Team will accept referrals from any source whereby there is concern for an individual’s mental health.

How should a referral be made?

Self-referral, family, carers should contact the Crisis Care Centre via telephone number 0800 0 328 728 (option 1) where possible in order to triage and arrange assessment venue and time appropriate to an individual’s needs.

Professional referrals, either urgent or routine, should contact the Crisis Care Centre via telephone number 0800 0 328 728 (option 1) with the exception of routine GP referrals that are made via ERS.

Self-presentations are also accepted. A triage assessment will be undertaken in order to determine an individual’s needs, whereby a further assessment may be arranged at a venue and time appropriate to an individual’s needs.

About the service

The Autism Spectrum Assessment Service is a screening, assessment and diagnosis service only.  Although we are unable to offer ongoing intervention, we do signpost to other appropriate services.

The service is for adults who do not have a learning disability but are experiencing symptoms and difficulties suggestive of an autism spectrum disorder. New referrals for those not known to mental health services are via the GP and can be sent directly to the Autism Spectrum Assessment Service at the above address.

Although crime is rarely committed as a direct result of mental ill-health, there is strong evidence which shows that offenders have complex and wide-ranging needs, including homelessness, drug and alcohol dependency, literacy and numeracy problems and mental health issues. The Criminal Justice Mental Health Team is a multi-agency mental health team which was set up to work within criminal justice settings in order to support addressing the needs of individuals with multiple complex vulnerabilities including health and social care needs.

The team carries out screening assessments in a variety of criminal justice settings such as courts, probation premises, prisons and police custody. The team’s primary role is to provide liaison between mental health and criminal justice services to ensure an individual’s needs are understood and taken into consideration as well as supporting diversion for those with needs to appropriate services to address identified needs and reduce potential for re-offending.

The team work with individuals throughout the criminal justice pathway from point of arrest right through to the end of any sentencing options, offering screening assessment, referrals, signposting and short-term support to address identified needs, support communication with criminal justice agencies and aid engagement with appropriate services.

They are a daily presence in the North Staffordshire Justice Centre (Monday to Friday), weekly clinics within local probation offices, probation approved premises and community rehabilitation companies. They provide a link to local prisons to support those who are being released from prison to have their mental health needs addressed and support enhanced risk management within the Trust.

They also support community mental health teams by offering advice, liaison and in circumstances where complexity of the case necessitates co-working when known service users are involved with the criminal justice system.

The team’s core value is to ensure equal and equitable access for our service users to mental health services to improve health and break the cycle of offending.

Outcomes for the users of the service may include:

  • Improved communication between criminal justice agencies and health and social care agencies
  • Diversion from the criminal justice system into hospital
  • Referral to other health and social care providers
  • Reducing in re-offending through enhanced risk management including liaison with MAPPA® and expertise in the use of structured risk assessment tools.
  • Intensive, time limited co-working; to enhance transition arrangements (e.g. on release from prison)

About the service

The Early Intervention Team is a dedicated service for those aged 14-64 years. It supports service users and families affected by a first episode of psychosis or at risk mental states. The team are implementing the access and waiting times standard by delivering NICE concordant interventions in a timely way.

About the service

The integrated Health and Social Care Community Mental Health Teams provide mental health and social care services to adults across North Staffordshire.

The services provided are based upon a recovery orientated model which enables adults with mental health related issues and their families, friends and significant others to live and maintain their optimum social roles.

Assessment , treatment and care is provided through a process known as Care Coordination. Services provided include:

  • Advice
  • Support
  • Assessment of mental health and social care needs
  • Treatment and care for people and their carers who have identified mental health needs which cannot be met in primary care.

The service is for adults of working age (18-75) with mental health-related difficulties, we transfer to older adults at 75. If someone is 65 and have not been known to mental health services before this referral goes to Neuro and Old Age Psychiatry.

About the service

This supported accommodation and resettlement service is delivered by a team of Support, Time and Recovery (STR) Workers. Tenancy support is provided by Brighter Futures who are the social care landlords.

Accommodation consists of shared supported tenancies offering 40 bed spaces in seven different locations within Stoke-on-Trent. The service is able to provide a variety of accommodation and support options depending upon individuals assessed needs. Occupational therapy assessments can be arranged.

A short break service is also available. The team provides support to carers and has an established carers group. The service works in partnership and within care coordination to provide person centred and needs led support that is recovery focused.

The team enables access to a range of educational, leisure, vocational, therapeutic and employment opportunities in order to support individuals towards independence

The service works to deliver collaborative treatment to those experiencing eating disorders within North Staffordshire.

Step On supports people who have experienced mental health difficulties, but who are now ready to return to or look for paid employment. The team work with service users in Staffordshire involved with mental health services. It is made up of skilled employment specialists and peer support workers who provide advice and support on possible career opportunities, focusing on a person’s abilities and aspirations.

Step On employment specialists work closely with employers to identify job opportunities and to match suitably skilled candidates.

Employment is a key feature of social inclusion. A person in work enjoys better health and wellbeing and greater financial independence. They also contribute more to the community and to the local economy.

People must be referred for IPS through their care coordinator, and must be ready to look for work.

About the service

The Assessment and Treatment Unit provides specialist interventions for community based clients who require short term support as a result of acute health care needs. It has five in-patient places offering short term assessment and treatment for a maximum of up to six months.

Short term assessment and treatment is provided for individuals when community placements have broken down due to acute health care needs. An in-depth assessment is carried out and an individually designed person centred plan or care programme is developed, to enable the client to return to their own home and be supported successfully within the community.

Who is eligible for this service?

Referrals are considered for adults with a learning disability who reside in or originate from North Staffordshire who require in-patient admission due to an acute breakdown in their healthcare needs.

The service will also take ‘out of area’ referrals where there has been an initial assessment undertaken to determine needs and treatment and the funding for the placement has been agreed.

Referrals routes

All referrals for admission will be made through the clients responsible Consultant Psychiatrist, who will request a multi-disciplinary admissions panel to consider the request for admission. Individuals must require emergency health care due to the breakdown of their ordinary community placement as a result of experiencing acute health care needs in relation to, severe and frequent challenging behaviours, acute mental health needs.

The Community Learning Disability Team is a specialist health service for people with a Learning Disability and their carers.  We support people who live in North Staffordshire and Stoke-on-Trent.

We provide specialist health assessments, support and care for people aged 18 and above.  We do this by working alongside other health and social care services in our area, to address the physical and mental health needs of a person with a Learning Disability.  We also provide support and advice to families and carers.
